Employees were removing a loading arm from the loading platform and were using a rope to hold the loading arm in place. The rope snapped, which caused the arm to swing down and hit an employee in the chest. The blow knocked him off of the platform and onto the concrete below, a distance of approximately 11.5'. He fractured his neck vertebra and skull.

An employee was assisting a mechanic in disassembling a gear box. The employee was holding an intermediate assembly inside the gear box when it shifted and created a pinch point between the assembly and the internal bore. The tip of one of the employee's index fingers was amputated.
